To request a name update, click ‘Settings’ on the left-hand side it will direct you to your account settings, where you can select ‘Edit.’ You will be able to click ‘Request name change.’ This will take you to a screen to enter the new First and Last name as well as upload documentation for the change. Once you submit the request, Ethos will reach out to you with additional forms and steps to complete the name change.
